2. Parent/Guardian Name – Include name of parent or guardian if registering children under age 18.
3. Email – Include full email address.
4. Contact Phone Number – Include home or cell number.
5. Student Age Range – Select appropriate age range.
6. Home Church – Include your home church.
7. Have you accepted Jesus Christ? – Check the appropriate box. 
8. Have you been baptized by immersion? – Check the appropriate box. 
9. Would you like to become a member of Chantilly Baptist Church? – Check the appropriate box.
